ABSTRACT

Este artigo tem como objetivo avaliar o acesso à prótese dentária na Atenção Primária em Saúde (APS) no Brasil. É um estudo transversal, em que foram utilizados dados do Programa de Melhoria do Acesso e da Qualidade Atenção Básica (PMAQ-AB), segundo e terceiro ciclos, com enfoque em saúde bucal e atendimento à prótese dentária por estado brasileiro. Ao comparar as Unidades Básicas de Saúde (UBS) que fazem instalações de prótese nos dois ciclos de avaliação, foi observada diferença estatisticamente significativa (teste Mann Whitney, p = 0,04), mostrando maior número de instalações no terceiro ciclo. O percentual de UBS que instalavam prótese dentária no segundo ciclo foi de 8,4% e no terceiro ciclo, 14,1%. Ao se avaliar os locais em que os pacientes fizeram a prótese dentária, verifica-se que a maioria fez suas próteses no serviço privado. Conclui-se, então, que a APS não consegue suprir a demanda por prótese dentária.


This cross-sectional study evaluates access to dental prosthesis in Primary Health Care (PHC) in Brazil. Data were obtained from the Access and Quality Improvement Program (PMAQ-AB), second and third cycles, focusing on oral health and dental care provided by the Brazilian State. Comparison of the Basic Health Units (BHU) that perform prosthesis installations in the two evaluation cycles (Mann Whitney test, p = 0.04) revealed a statistically significant difference, showing a greater number of installations in the third cycle. In the second and third cycles 8.4% and 14.1% of UBS, respectively, installed dental prostheses. When evaluating where patients acquired their dental prosthesis, most sought the private service. In conclusion, PHC cannot meet the demand for dental prosthesis.


Este artículo tuvo por objetivo evaluar el acceso a las prótesis dentales en la atención primaria de salud (APS) en Brasil. Se trata de un estudio transversal que utiliza datos del Programa de Mejoramiento del Acceso y Calidad de la Atención Primaria (PMAQ-AB, por sus siglas en portugués), segundo y tercer ciclo, con foco en salud bucal y atención con prótesis dental por estado brasileño. Al comparar las Unidades Básicas de Salud (UBS) que instalan prótesis en los dos ciclos de evaluación, se observó diferencia estadísticamente significativa (prueba de Mann Whitney, p = 0,04), con un mayor número de instalaciones en el tercer ciclo. El 8,4% de las UBS instalaron prótesis dental en el segundo ciclo, y el 14,1% en el tercer ciclo. En cuanto a los locales donde los pacientes hicieron su prótesis dental, la mayoría fue realizada por el servicio privado. Se concluye que la APS no logra satisfacer la demanda de prótesis dentales.

ABSTRACT

Aim: This study aimed to investigate the relationship between self-perceived oral health and the prosthetic status of individuals who seek care in prosthodontics clinics. Methods: Self-perceived oral health was analyzed through the Geriatric Oral Health Assessment Index (GOHAI). Abutment tooth mobility was assessed, and denture status was determined by clinical assessment of stability, retention, occlusal balance, vertical dimension, and integrity of dentures. The frequency of individuals in each variable was determined for the low and very low GOHAI conditions. Chi-square, Pearson, and stepwise logistic regression tests were used for the statistical analyses (α = 0.05). Results: Ninety wearers of removable dentures with a mean age of 55.1±9.1 years were evaluated. None of the variables was related to GOHAI values (p > 0.05). The regression analysis showed that age predicts (p = 0.006) the variation of GOHAI conditions (OR = 0.924, CI = 0.873- 0.978), showing only 7.6% protective effect against very low GOHAI. Unsatisfactory stability showed the opposite effect (p = 0.034) and the individuals with this characteristic are more likely (OR = 3.055) to have very low GOHAI scores (CI = 1.085- 8.602). Conclusions: The self-perceived oral health of wearers of removable dentures worsens with age and unsatisfactory stability of dentures

ABSTRACT

La tecnología CAD/CAM es una forma innovadora relacionada con la implementación de técnicas digitales para mejorar las restauraciones prostéticas en un paciente, entre ellas, las destinadas a la estética dental. El presente estudio tiene el objetivo de describir la utilidad de la tecnología CAD/CAM en la fabricación de prótesis dentales. Para ello se realizó una revisión de la literatura médica disponible en las bases de datos PubMed/Medline, Scopus y SciELO, mediante fórmulas de búsqueda avanzada; se seleccionaron artículos en idioma español e inglés. En la actualidad, con esta tecnología se ha logrado combinar estética, resistencia y precisión en la restauración directa, además de garantizarse una mayor duración y años de funcionalidad para cada una de las prótesis, coronas e implantes. La calidad de vida y satisfacción referidas por los pacientes portadores de piezas producidas con esta tecnología, es superior a las manifestadas por aquellos tratados con piezas de fabricación tradicional. El uso de técnicas de inteligencia artificial y biomateriales representará un cambio cualitativo y cuantitativo en el uso de los sistemas CAD/CAM.


CAD/CAM technology is an innovative way related to the implementation of digital techniques to improve prosthetic restorations in a patient, including those intended for dental aesthetics. The objective of this study is to describe the usefulness of CAD/CAM technology in the dental prostheses manufacture. A review of the medical literature available in the PubMed/Medline, Scopus and SciELO databases was carried out, using advanced search formulas; Articles in Spanish and English were selected. Currently, with this technology it has been possible to combine aesthetics, resistance and precision in direct restoration, in addition to guaranteeing greater duration and years of functionality for each of the prostheses, crowns and implants. The quality of life and satisfaction reported by patients wearing parts produced with this technology is higher than those reported by those treated with traditionally manufactured parts. The use of artificial intelligence and biomaterials techniques will represent a qualitative and quantitative change in the use of CAD/CAM systems.

ABSTRACT

A utilização da tecnologia CAD/CAM (computer aided design/computer aided manufacturing) e 3D (tridimensional) para reconstruções alveolares na implantodontia permite o planejamento pré-operatório detalhado, o design do resultado desejado do enxerto e a avaliação virtual do resultado em relação à reconstrução protética. Este trabalho objetiva detalhar a técnica cirúrgica de enxerto ósseo sintético confeccionado por meio dessa tecnologia, seguido da instalação de implantes osseointegráveis e reabilitação protética em um defeito ósseo na mandíbula. Paciente masculino, 22 anos, compareceu à clínica por avulsão dos elementos 32, 31, 41 e 42 com significativa perda óssea vertical de rebordo alveolar. Devido à extensa perda óssea, realizou-se enxerto em bloco prototipado impresso. O escaneamento intraoral e os arquivos de imagem gerados foram enviados ao centro de planejamento virtual. Primeiramente, foi realizada a cirurgia para instalação do enxerto em bloco. Após o período de cicatrização, foi realizada cirurgia para instalação de dois implantes osseointegráveis. Esperado o período de três meses a partir da instalação dos implantes, foi realizada a confecção de próteses provisórias fixas sobre implantes a fim de realizar o condicionamento dos tecidos moles peri-implantares e o carregamento progressivo dos implantes. O aumento do rebordo alveolar através do uso de enxerto em bloco impresso personalizado apresentou-se como uma técnica com inúmeras vantagens, por não necessitar de sítio doador, reduzir tempo cirúrgico e apresentar perfeita adaptação do bloco ao defeito ósseo, resultando em menor morbidade pós-operatória. Essa técnica é uma indicação para casos de defeitos ósseos severos, visando a otimizar o resultado e a propiciar menor desconforto ao paciente.


The use of the technology CAD/CAM (computer aided design/computer aided manufacturing) and 3D (three dimensional) to alveolar reconstructions in implant dentistry allows detailed preoperative planning, the design of the desired grafting result, and the virtual evaluation of the result in relation to the prosthetic reconstruction. This paper aims to details the synthetic bone graft made through this technology, followed by the installation of osseointegrated implants and prosthetic rehabilitation in a bone imperfection in the jaw. A 22-year-old man attended the clinic due to a tooth avulsion of the four lower incisors with significant vertical bone loss of alveolar ridge. Because of the extensive bone loss, it was made a prototyped printed block graft. The intraoral scan and the generated image files were sent to the virtual planning center. First, it was necessary a surgery to install the block graft. After the healing process, it was made a surgery to put two osseointegrated implants. Three months later, temporary fixed prostheses on implants were made to conditioning the peri-implant soft tissues and the progressive loading of the implants. The increase of the alveolar ridge using personalized printed block graft was presented as a technique with numerous advantages, since it does not require a donor site, reduces the surgical time and presents perfect adaptation of the block to the bone imperfection, resulting in a less postoperative morbidity. This technique is indicated to cases of severe bone imperfections, aims to optimize results, and provide less discomfort to the patient.

En la actualidad es cada vez más frecuente la consulta de pacientes que siendo sometidos en edades tempranas a cirugías de extirpación tumoral y posterior reconstrucción con injertos presentan problemas tanto funcionales como estéticos, el uso de prótesis implantosoportadas mejora la calidad de vida, pero al mismo tiempo representan un desafío por la baja disponibilidad ósea. Se presenta un caso clínico de rehabilitación protésica implantosoportada en una paciente con gran déficit óseo mandibular como consecuencia de le extirpación de una neoplasia y posterior injerto de costilla. Se colocaron 2 implantes osteointegrados en hueso remanente y rehabilitación con prótesis removible sostenida a una barra colada. Por ser un tratamiento poco invasivo y conservador fue aceptado fácilmente por la paciente y la mejora tanto en la estética como en la función fue notoria.


Nowadays, it is becoming more and more frequent to see patients who underwent surgery for tumor removal and subsequent reconstruction with grafts at an early age, presenting both functional and esthetic problems. The use of implant-supported prostheses improves the quality of life, but at the same time represents a challenge due to the low availability of bone. A clinical case of implant-supported prosthetic rehabilitation is presented in a patient with a great mandibular bone deficit as a consequence of the removal of a neoplasm and subsequent rib graft. Two osseointegrated implants were placed in the remaining bone and rehabilitation with a removable prosthesis supported by a cast bar. Being a minimally invasive and conservative treatment, it was easily accepted by the patient and the improvement in both esthetics and function was notorious.


Atualmente, são cada vez mais frequentes os pacientes submetidos à cirurgia de remoção de tumores e posterior reconstrução com enxertos em idade precoce que apresentam problemas funcionais e estéticos. O uso de próteses implanto-suportadas melhora a qualidade de vida, mas, ao mesmo tempo, representa um desafio devido à baixa disponibilidade de osso. Apresentamos um caso clínico de reabilitação protética implanto-suportada em um paciente com grande déficit ósseo mandibular em consequência da remoção de uma neoplasia e posterior enxerto de costela. Dois implantes osseointegrados foram colocados no osso remanescente e a reabilitação foi feita com uma prótese removível suportada por uma barra de gesso. Por se tratar de um tratamento minimamente invasivo e conservador, foi facilmente aceito pelo paciente e a melhora na estética e na função foi perceptível.

ABSTRACT Based on its mechanical properties, PEEK (polyether-ether-ketone) might be useful in restorative procedures. In oral rehabilitation, its viability has been studied mainly for prostheses and dental implants. Aim: The aim of this study was to evaluate the fit accuracy of dental implant bars made of either PEEK or cobalt-chrome submitted to cycling mechanics. Materials and Method: This was an experimental in vitro study, where units were treated with two implants and mini-abutments, joined by cobalt-chrome or polyether-ether-ketone PEEK bars. A total 20 bars were prepared (n=10 per group) and subjected to mechanical cycling tests (1 million cycles on the distal cantilever of the bar in the vertical direction, 120N and sinusoidal loading, at a frequency of 2Hz). The fit at the abutment/implant interface was measured before and after cycling, and the counter-torque of the vertical screw of the mini abutments was measured after cycling, using a digital torquemeter. Data were analyzed by three-way ANOVA and Tukey's test at 5% significance level. Results: No statistically significant interaction was found among the three factors considered (bar material, implant positioning and mechanical cycling) (p = 0.592). No significant difference was identified in the interaction between bar material and implant positioning (p = 0.321), or between implant positioning and mechanical cycling (p = 0.503). The association between bar material and mechanical cycling was statistically significant (p = 0.007), with the cobalt-chrome bar resulting in greater misfit with mechanical cycling. There was no difference in counter-torque values between groups. Conclusions: The PEEK bar provided better fit of the mini abutments to the implants, even after mechanical cycling. The counter-torque of the screws was similar in all scenarios considered.


RESUMO O PEEK (Poli-éter-éter-cetona) é um material considerado para uso em procedimentos restauradores devido às suas propriedades mecânicas. Na reabilitação oral, sua viabilidade tem sido estudada principalmente para uso em próteses e implantes dentários. Objetivos: O objetivo deste estudo foi avaliar a precisão da adaptação de duas barras diferentes do tipo protocolo confeccionadas em PEEK ou Cobalto-Cromo, após serem submetidas à mecânica ciclística. Materiais e Método: As unidades experimentais foram constituídas por barras confeccionadas em Poli-ether-ether-Ketone (PEEK) e em Cobalto-Cromo (Co-Cr). Trata-se de um estudo experimental, in vitro, onde verificou-se unidades constituídas por dois implantes e mini pilares unidos com barras de Cobalto-Cromo ou PEEK. Foram confeccionados um total de 20 barras (n=10 em cada grupo) e as barras foram submetidas a ensaios de ciclagem mecânica (1 milhão de ciclos no cantilever distal da barra no sentido vertical, 120N e carregamento senoidal, a uma frequência de 2Hz). Antes e após a ciclagem realizou-se a mensuração da desadaptação na interface pilar/implante e após a ciclagem foi medido o contra-torque do parafuso vertical dos mini-pilares através de torquímetro digital TQ 8800 (LT Lutron, Taiwan). Os dados foram submetidos a ANOVA a três critérios e teste de Tukey ao nível de significância a 5%. Resultados: Constatou-se que não houve interação estatisticamente significativa entre os três fatores estudados, ou seja, entre o material da barra, o posicionamento do implante e a ciclagem mecânica (p = 0,592). Também não se identificou diferença estatística significativa da interação entre o material da barra e o posicionamento do implante (p = 0,321), nem entre o posicionamento do implante e a ciclagem mecânica (p = 0,503). Já a associação entre o material da barra e a ciclagem mecânica foi estatisticamente significativa (p = 0,007), onde a barra de Cobalto-Cromo resultou em maior desadaptação com a ciclagem mecânica. Não houve diferença nos valores dos contra-torques entre os grupos. Conclusões: Conclui-se que a barra de protocolo fabricada em PEEK proporcionou melhor adaptação dos mini pilares aos implantes mesmo após a ciclagem mecânica. Por fim, o contra-torque dos parafusos foi semelhante em todos os cenários avaliados.

ABSTRACT

As próteses parciais removíveis (PPRs) são uma alternativa de tratamento viável na prática clínica para reabilitar arcos parcialmente desdentados. Entretanto, o planejamento dessas próteses e preparo bucal prévio são frequentemente negligenciados. O presente artigo tem como objetivo relatar o caso clínico de uma reabilitação oral com PPRs superior e inferior após abordagem multidisciplinar, de modo a enfatizar as fases de um planejamento criterioso e de preparo prévio dos dentes pilares, visando o sucesso e a longevidade da reabilitação. Paciente do sexo masculino de 57 anos de idade compareceu à clínica da Faculdade de Odontologia de Bauru, Universidade de São Paulo, queixando-se da estética de seu sorriso e de algumas ausências dentárias. O indivíduo utilizava uma PPR provisória inferior insatisfatória e apresentava perda de dimensão vertical de oclusão (DVO). Após o exame clínico, radiográfico e estudo do caso em articulador semi-ajustável, realizou-se o planejamento com abordagens restauradoras, endodônticas, periodontais e protéticas. Após tratamento periodontal, foi realizada endodontia dos elementos 12, 15 e 47, confecção de núcleos e coroas nos dentes 12 e 15, restauração a nível gengival para apoio residual no dente 47, restaurações nos dentes 11, 13, 22, 24 e 44 e, por fim, a confecção das PPRs superior e inferior. A abordagem multidisciplinar utilizada neste caso clínico viabilizou o restabelecimento da DVO e possibilitou o sucesso da reabilitação protética(AU)


Removable partial dentures (RPDs) are a viable treatment alternative in clinical practice to rehabilitate partially edentulous arches. However, the planning of these dentures and prior oral preparation are often neglected. This article aims to report the clinical case of an oral rehabilitation with upper and lower RPDs after a multidisciplinary approach, to emphasize the phases of careful planning and prior preparation of the abutment teeth, aiming at the success and longevity of the rehabilitation. A 57-year-old male patient came to the clinic of the Bauru School of Dentistry, University of São Paulo, complaining about the esthetics of his smile and some missing teeth. The subject was using an unsatisfactory lower provisional prosthesis and had a loss of vertical dimension of occlusion (OVD). After the clinical and radiographic examination and the case study in a semi-adjustable articulator, planning was carried out with restorative, endodontic, periodontal and prosthetic approaches. After periodontal treatment, endodontics were performed on elements 12, 15 and 47, creation of cores and crowns on teeth 12 and 15, restoration at the gingival level for residual support on tooth 47, restorations on teeth 11, 13, 22, 24 and 44 and, finally, the making of the upper and lower PPRs. The multidisciplinary approach used in this clinical case enabled the restoration of the OVD and enabled the success of the oral rehabilitation(AU)

ABSTRACT

O envelhecimento acarreta mudanças em todos os aspectos da vida, sendo caracterizado por uma modificação nas funções sociais, mentais e corporais. No Sistema Estomatoglossognático, é possível observar alterações significativas, começando pela fase preparatória da deglutição e da função de mastigação, decorrente da perda dos dentes, a qual é responsável pela trituração do alimento, formação do bolo alimentar e perda do tônus dos músculos que são responsáveis pelos movimentos mastigatórios. Esta pesquisa observou como as alterações dentárias em idosos podem repercutir na sua alimentação. Foi realizado um estudo observacional transversal, com amostra de 33 idosos frequentadores da Igreja Evangélica Assembleia de Deus em Alagoas, no município de Coqueiro Seco. Foi feita aplicação de um instrumento de coleta, o qual possuía perguntas baseadas no MBGR e na escala EAT-10. Os resultados mostraram que há um grande número de idosos que fazem uso de prótese dentária, refletindo na execução da função mastigatória, escolha da consistência alimentar e aspectos socioemocionais. As próteses dentárias, quando bem adaptadas, podem gerar um ganho significativo ao paciente, permitindo uma alimentação que não interfere na sua postura social, nem em suas questões emocionais. Aging causes changes in all aspects of life, being characterized by a change in social, mental and bodily functions. In the Stomatoglossognathic System, it is possible to observe significant changes, starting with the preparatory phase of swallowing and the chewing function, resulting from the loss of teeth; which is responsible for crushing food, formation of food bolus and loss of muscle tone that are responsible for masticatory movements. This research observed how dental changes in the elderly can affect their diet. A cross-sectional observational study was carried out, with a sample of 33 elderly people attending the Evangelical Assembly of God Church in Alagoas, in the municipality of Coqueiro Seco. A collection instrument was applied, which had questions based on the MBGR and the EAT-10 scale. The results showed that there is a large number of elderly people who use dental prosthesis, reflecting on the performance of masticatory function, choice of food consistency and socio-emotional aspects. Dental prostheses, when well adapted, can generate a significant gain for the patient, allowing a diet that does not interfere with their social posture, nor with their emotional issues. (AU)


El envejecimiento provoca cambios en todos los aspectos de la vida, caracterizándose por un cambio en las funciones sociales, mentales y corporales. En el Sistema Estomatoglosognático, es posible observar cambios significativos, a partir de la fase preparatoria de la deglución y de la función masticatoria, resultantes de la pérdida de dientes; que se encarga de triturar los alimentos, formación de bolos alimenticios y pérdida de tono muscular que son responsables de los movimientos masticatorios. Esta investigación observó cómo las alteraciones dentales en los adultos mayores pueden afectar su alimentación. Se realizó un estudio observacional transversal, con una muestra de 33 ancianos asistentes a la Iglesia Evangélica Asamblea de Dios de Alagoas, en el municipio de Coqueiro Seco. Se aplicó un instrumento de recolección, el cual contó con preguntas basadas en el MBGR y la escala EAT-10. Los resultados mostraron que existe un gran número de ancianos que utilizan prótesis dental, reflexionando sobre el desempeño de la función masticatoria, elección de la consistencia de los alimentos y aspectos socioemocionales. Las prótesis dentales, bien adaptadas, pueden generar una ganancia importante para el paciente, permitiendo una alimentación que no interfiere con su postura social, ni con sus problemas emocionales. ABSTRACT

The development of dental implants has revolutionized the rehabilitation possibilities for partially or totally edentulous patients. The long-term success of the dental implant depends on osseointegration and this is achieved by the combination of two essential stabilities: primary and secondary. Implant-supported prostheses are a great evolution for these treatments, positively interfering in the quality of life of the population, as they generate function and aesthetics for the patient. Thus, the present study aimed to radiographically evaluate complete dentures supported by an external hexagon conical implant in the anterior region of the mandible. More specifically, to evaluate the feasibility of using these implants in cortical bone of the mandible, to verify the success rate regarding the significance of peri-implant bone loss and to compare peri-implant bone loss between patients.

ABSTRACT

Aim: To describe the burden of oral diseases and of self-reported periodontal disease of patients under Oral Anticoagulation Therapy (OAT) with warfarin. Methods: A cross-sectional study was conducted. Validated questionnaires assessed self-reported periodontal disease and demographic variables. After calibration (Kappa > 0.80), an examiner evaluated dental caries and the need for dental prostheses. Statistical analysis involved proportions and measures of central tendency. Results: The sample consisted of 158 individuals, with a mean age of 58.8 years (SD = 12.1), of which 62.7% of the participants were women. The average DMFT (Decayed, Missing, and Filled Teeth) index was 22.9 (SD = 7.6), with the missing component being the highest (Mean = 16.23). The use of maxillary prosthesis (53.2%) was higher than mandibular (32.3%). The need for mandibular prosthesis reached 66.5%. The percentage of participants that referred gum disease, tooth migration, and tooth mobility was 29.6%, 37.4%, and 30.4%, respectively. Conclusions: The burden of oral diseases among individuals undergoing OAT is worrisome.


Objetivo: Describir la carga de enfermedades bucales y la enfermedad periodontal autorreportada de pacientes en tratamiento con anticoagulación oral con warfarina. Métodos: Se realizó un estudio transversal. Los cuestionarios validados evaluaron la enfermedad periodontal autoinformada y las variables demográficas. Después de la calibración (Kappa> 0.80), un examinador evaluó la caries dental y la necesidad de prótesis dentales. El análisis estadístico involucró proporciones y medidas de tendencia central. Resultados: La muestra estuvo formada por 158 individuos, con una edad media de 58.8 años (DE = 12.1), de los cuales el 62.7% de los participantes eran mujeres. El índice CPOD promedio fue de 22.9 (DE = 7.6), siendo el componente perdido el que más contribuyó al índice (Media = 16.23). El uso de prótesis maxilar (53.2%) fue mayor que el de prótesis mandibular (32.3%). La necesidad de prótesis mandibular alcanzó el 66.5%. El porcentaje de participantes que informaron enfermedad de las encías, migración de los dientes y movilidad de los dientes fue del 29.6%, 37.4% y 30.4%, respectivamente. Conclusiones: Las enfermedades bucales y la necesidad de rehabilitación oral entre los individuos sometidos a anticoagulación oral con warfarina fue motivo de preocupación.

ABSTRACT

Inflammatory fibrous hyperplasia (IFH) is a common reactive lesion in dental prostheses users that may be associated with chondroid metaplasia (CM). Metaplasia is an adaptive cellular process that may be caused by trauma. We reported here five cases of IFH associated with CM and analyzed morphologically the deposition of collagen in these lesions. Patients had a mean age of 58.8 years-old and were ill-fitting dental prostheses users. They presented nodular lesions located in the anterior maxilla. Microscopically, it was observed hyperplastic fibrous connective tissue with chronic inflammatory infiltrate and hyaline cartilage. No morphological differences were observed in collagen deposition under light microscopy, but quantitative analysis revealed a significantly higher collagen deposition at the connective tissue near CM (p = 0.015). IFH associated with CM affects ill-fitting dental prostheses users. The presence of CM is not significant to the lesion prognosis. However, its formation and the higher collagen deposition near it reinforces the IFH reactive origin.


La hiperplasia fibrosa inflamatoria (HFI) es una lesión reactiva común en los usuarios de prótesis dentales que puede estar asociada con la metaplasia cartilaginosa (MC). La metaplasia es un proceso celular adaptativo que puede ser causado por un trauma. El presente informe analizó cinco casos de HFI asociados a MC y se analizaron morfológicamente la deposición de colágeno en estas lesiones. Los pacientes tenían una edad media de 58,8 años y eran usuarios de prótesis dentales mal adaptadas. Se observaron lesiones nodulares localizadas en el la parte anterior del maxilar Microscópicamente se observó tejido conectivo fibroso hiperplásico con infiltrado inflamatorio crónico y cartílago hialino. No se observaron diferencias morfológicas en la deposición de colágeno bajo microscopía óptica, pero el análisis cuantitativo reveló una deposición de colágeno significativamente mayor en el tejido conectivo cerca de MC (p = 0,015). La HFI asociada con la MC afecta a los usuarios de prótesis dentales mal adaptadas. La presencia de MC no es significativa para el pronóstico de la lesión. Sin embargo, su formación y la mayor deposición de colágeno cerca de MC refuerza el origen reactivo de HFI.

ABSTRACT

This study evaluated the dental prosthesis needs and associated factors in indigenous Brazilian adults from Northeastern Brazil. Dental prosthesis needs were assessed in a representative sample of 225 Indians. Edentulous area and current indication for tooth extraction according to periodontal status or non-restorable condition were registered in a clinical chart. Bivariate and logistic analyses were applied to assess associations of dental prosthesis needs and prosthesis type with age, sex, income, education, smoking, moderate or severe periodontitis, plaque index, dental caries experience, and previous dental visit. Prosthetic treatment was necessary in 83% of the participants. These individuals required 339 dental prostheses, including removable partial dentures (60.2%), fixed (33.6%), and complete dentures (6.2%). Regression analysis showed that age ≥35 years [odds ratio (OR) 5.95, 95% confidence interval (CI) 1.97­17.91, p=0.002] and caries experience ≥3 [OR 4.01, 95%CI 1.78­9.02, p=0.001] were significantly associated with prosthesis needs. The type of prosthesis required was associated with sex, age, educational level, periodontitis, caries experience, and plaque index (p≤0.03). In conclusion, the need for prosthetic rehabilitation was high in this population, and sociodemographic factors, dental plaque level, caries experience, and periodontitis were associated with the required type of prosthesis. These data can be used to plan the allocation of resources to promote preventive and curative strategies in this population.(AU)


Este estudo avaliou as necessidades de prótese dentária e fatores associados em adultos indígenas brasileiros do Nordeste do Brasil. As necessidades de prótese dentária foram avaliadas em uma amostra representativa de 225 índios. A área edêntula e a indicação para extração dental, de acordo com o estado periodontal ou condição não restaurável, foram registradas em um prontuário clínico. Análises bivariadas e logísticas foram aplicadas para avaliar as associações das necessidades de prótese dentária e tipo de prótese com idade, sexo, renda, escolaridade, tabagismo, periodontite moderada ou grave, índice de placa, experiência de cárie dentária e consulta odontológica anterior. O tratamento protético foi necessário em 83% dos participantes. Esses indivíduos necessitaram de 339 próteses dentárias, incluindo próteses parciais removíveis (60,2%), fixas (33,6%) e próteses totais (6,2%). A análise de regressão mostrou que idade ≥35 anos [odds ratio (OR) 5,95, intervalo de confiança de 95% (IC) 1,97­17,91, p=0,002] e experiência de cárie ≥3 [OR 4,01, IC 95% 1,78­9,02, p=0,001] foram significativamente associados às necessidades de prótese. O tipo de prótese necessária foi associado ao sexo, idade, escolaridade, periodontite, experiência de cárie e índice de placa (p≤0,02). Em conclusão, a necessidade de reabilitação protética foi alta nesta população e fatores sociodemográficos, nível de placa dentária, experiência de cárie e periodontite foram associados ao tipo de prótese necessária. Estes dados podem ser usados para planejar a alocação de recursos para promover estratégias preventivas e curativas nessa população.(AU)

ABSTRACT Objective: Evaluated the antifungal effect of the incorporation of different concentrations of the essential oil Cymbopogon citratus (capim santo), into polymethylmethacrylate (PMMA) against Candida albicans. Methods: Fifty specimens were fabricated and divided into five groups: Group 1, PMMA + 10% essential oil (n=10); Group 2, PMMA + 15% essential oil (n=10); Group 3, PMMA + 20% essential oil (n=10); Group 4, PMMA + 25% essential oil (n=10); Group 5, PMMA (n=10). PMMA powder was mixed with the monomer and the mixture was placed in disc-shaped cavities measuring 15 mm in diameter, 2 mm thick. To evaluate the antifungal activity of the experimental specimens, the standard strain of Candida albicans was tested. After incubation, the colony count of each plate was performed using a digital colony counter, obtaining the number of colony forming units (CFU) and the Kruskal-Wallis test was applied. Results: There was statistically significant difference in the CFU count of Candida albicans as a consequence of the addition of Cymbopogon citratus essential oil to PMMA (p < 0.001) and values were significantly higher in comparison with those of all the other groups, when the essential oil was incorporated as incorporated into the PMMA in the concentration of 20%. In the other concentrations, no difference in values was observed in comparison with the Control Group without essential oil of Cymbopogon citratus. Conclusion: The acrylic resin with the essential oil incorporated into it in different concentrations provided no effect against development of the genus Candida.


RESUMO Objetivo: Avaliar o efeito antifúngico da incorporação de diferentes concentrações do óleo essencial Cymbopogon citratus (capim santo), em polimetilmetacrilato (PMMA) contra Candida albicans. Métodos: Cinquenta corpos de prova foram confeccionados e divididos em cinco grupos: Grupo 1, PMMA + 10% de óleo essencial (n=10); Grupo 2, PMMA + 15% de óleo essencial (n=10); Grupo 3, PMMA + 20% de óleo essencial (n=10); Grupo 4, PMMA + 25% de óleo essencial (n=10); Grupo 5, PMMA (n=10). O pó de PMMA foi misturado ao monômero e a mistura foi colocada em cavidades em forma de disco medindo 15 mm de diâmetro por 2 mm de espessura. Para avaliar a atividade antifúngica dos espécimes experimentais, foi testada a cepa padrão de Candida albicans. Após a incubação, foi realizada a contagem de colônias de cada placa por meio de um contador digital de colônias, obtendo-se o número de unidades formadoras de colônias (UFC) e para isso foi aplicado o teste de Kruskal-Wallis. Resultados: Houve diferença estatisticamente significativa na contagem de UFC de Candida albicans como consequência da adição do óleo essencial de Cymbopogon citratus ao PMMA (p < 0,001) e os valores foram significativamente maiores em comparação com todos os outros grupos, quando o essencial óleo foi incorporado como incorporado ao PMMA na concentração de 20%. Nas demais concentrações, não houve diferença nos valores em relação ao Grupo Controle sem óleo essencial de Cymbopogon citratus. Conclusão: A resina acrílica com o óleo essencial incorporado a ela em diferentes concentrações não apresentou efeito contra o desenvolvimento do gênero Candida.

ABSTRACT Purpose: to assess the influence of dentures on the electromyographic activity of masticatory muscles in individuals with Parkinson's disease. Methods: an analytical, quantitative, cross-sectional study conducted at a public teaching hospital in 2017, assessing the stage of the disease, non-denture (ND) wearing, upper removable partial denture wearing (DW1), upper and lower removable partial denture wearing (DW2), and upper complete denture + lower removable partial denture wearing (DW3). The Student-Newman-Keuls test was used to compare the maximum voluntary muscle contraction between sides and between the denture wearing and non-denture wearing groups. Significance was set at α<0.05. Results: after applying the eligibility criteria, 41 patient records were included in the sample. The association between the stage of the disease and denture wearing showed predominance of DW2 in stage 1 (43%). A significant difference was found in the masseter muscle analysis between the non-denture wearing group and DW2 and DW3 (p=0.0018). As for the anterior temporalis muscle, there was a significant difference (p=0.0034) in three of the analysis groups (ND vs. DW2, ND vs. DW3, and DW1 vs. DW3). Conclusion: denture wearing has a negative impact on the electrical activity of masticatory muscles when compared with individuals who do not wear dentures and have natural teeth.

Abstract In this study, we aimed to evaluate the halitosis and pain threshold of the peri-implant soft tissues in individuals rehabilitated with implant-supported prostheses. Forty-eight subjects were divided into four groups (n = 12) according to their prosthetic rehabilitation: single-tooth fixed prosthesis, multi-tooth fixed prosthesis, overdentures, and the Brånemark protocol. Halitosis was measured using a halimeter, whereas the pain threshold was measured using Von Frey monofilaments. Measurements were taken before (t0) and 30 days after (t1) placement of healing caps, and at the time of (t2) and 30 days after (t3) prosthetic placement. Halitosis data were analyzed using the chi-square test and Bonferroni correction (p < 0.05). Two-way ANOVA and Tukey's test (p < 0.05) were used to analyze pain threshold data. We noted an association between halitosis and time for the Brånemark protocol [X2(6) = 18.471; p = 0.005] and overdenture groups [X2(6) = 17.732; p = 0.007], and between halitosis and type of prosthesis only at t0 [X2(6) = 12.894; p = 0.045]. The interaction between time and the type of prosthesis significantly interfered with the mean pain threshold values (p = 0.001). At most time points, the majority of participants in each group had clinically unacceptable halitosis. After 30 days of using the prostheses, the overdenture group had a lower pain threshold compared to the Brånemark protocol group.


Resumo Este estudo teve como objetivo avaliar a halitose e o limiar de dor dos tecidos moles peri-implantares em indivíduos reabilitados com próteses implantossuportadas. Um total de 48 indivíduos foram divididos em quatro grupos (n=12), de acordo com as reabilitações: prótese fixa unitária, prótese fixa multidentária, sobredentadura e protocolo de Brånemark. A halitose foi medida com um halímetro, enquanto o limiar de dor foi medido com monofilamentos de von Frey. As medições foram feitas antes (t0) e 30 dias após (t1) a colocação das tampas de cicatrização e no momento (t2) e 30 dias após (t3) a colocação da prótese. Os dados de halitose foram analisados por meio do teste qui-quadrado e correção de Bonferroni (p < 0,05). ANOVA de duas vias e o teste de Tukey (p < 0,05) foram usados para analisar os dados do limiar de dor. Observou-se associação entre halitose e tempo para o protocolo de Brånemark [X2(6) = 18,471; p = 0,005] e grupos overdenture [X2(6) = 17,732; p = 0,007], e entre halitose e tipo de prótese apenas em t0 [X2(6) = 12,894; p = 0,045]. A interação entre o tempo e o tipo de prótese interferiu significativamente nos valores médios do limiar de dor (p = 0,001). Na maioria dos pontos de tempos, a maioria dos participantes de cada grupo apresentava halitose clinicamente inaceitável. Após 30 dias de uso das próteses, o grupo overdenture apresentou menor limiar de dor em comparação ao grupo do protocolo de Brånemark.

ABSTRACT

OBJECTIVE: This systematic review assessed the effects of prosthetic rehabilitation with removable dentures on oral health-related quality of life (OHRQoL) in older adults. METHODS: A comprehensive electronic search was conducted in the PubMed/MEDLINE, Web of Science, and Cochrane databases to identify randomized clinical trials and prospective clinical studies. The included studies evaluated the effects of prosthetic rehabilitation on OHRQoL in patients whose mean age was > 60 years. The interventions included complete dentures, implant-retained overdentures, and removable partial prostheses, with a minimum follow-up period of 1 month after prosthetic rehabilitation. Patient-reported outcome measures, specifically OHRQoL, were the primary outcome. Risk of bias was assessed using the Cochrane RoB 2 tool for randomized clinical trials and the ROBINS-I tool for prospective clinical studies. RESULTS: Eleven articles were considered eligible for the systematic review. The findings indicated that removable dentures improved various OHRQoL domains, mainly functional limitations and physical and psychological disabilities. Retention, stability, comfort, speech, and masticatory efficiency were significantly better with implant overdentures than complete dentures, leading to higher patient satisfaction and OHRQoL. CONCLUSIONS: This systematic review suggests that prosthetic rehabilitation with removable dentures has a positive influence on OHRQoL in older patients. The findings highlight the beneficial impact of implant overdentures and fixed adhesive prostheses for enhancing functional outcomes and patient satisfaction.


OBJETIVO: Esta revisão sistemática avaliou os efeitos da reabilitação protética com próteses removíveis na qualidade de vida relacionada à saúde bucal (QVRSB) em pacientes idosos. METODOLOGIA: Foi realizada uma pesquisa eletrônica abrangente nas bases de dados United States National Library of Medicine/ Medical Literature Analysis and Retrieval System Online ­ PubMed/MEDLINE, Web of Science e Cochrane para identificar ensaios clínicos randomizados (ECR) e estudos clínicos prospectivos. Os artigos selecionados tiveram como foco avaliar o efeito da reabilitação protética na QVRSB em indivíduos com média de idade superior a 60 anos. As intervenções descritas incluíram próteses totais, overdentures suportadas por implantes e próteses parciais removíveis, com acompanhamento mínimo de um mês após a reabilitação protética. As medidas de resultados relatadas pelos pacientes, especificamente QVRSB, foram o desfecho primário. O risco de viés foi avaliado com a ferramenta Cochrane RoB 2 pare ensaios clínicos randomizados e a ferramenta ROBINS-I para estudos clínicos prospectivos. RESULTADOS: Os resultados mostraram que as próteses dentárias removíveis melhoraram vários domínios da QVRSB, principalmente limitações funcionais e físicas e deficiências psicológicas. A retenção, estabilidade, conforto, fala e eficiência mastigatória foram significativamente melhores com overdentures sobre implantes do que com próteses totais, levando a maior satisfação do paciente e QVRSB. CONCLUSÕES: Esta revisão sistemática sugere uma influência positiva da reabilitação protética com próteses removíveis na QVRSB em pacientes idosos. As descobertas destacam o impacto benéfico das overdentures sobre implantes e das próteses adesivas fixas para melhorar os resultados funcionais e a satisfação do paciente

Aim: To analyze the accuracy of extraoral systems (Ceramill Map400+, AutoScan-DS200+, and E2) in full implantprosthetic rehabilitation three-dimensionally. Methods: A metallic edentulous maxilla with four implants was digitalized by a contact scanner (MDX-40 - Roland, control) and used as a control image to compare with other images generated by three laboratory scanners (10 samples per group). Letters identified all the four components: A and D angled 45º, and B and C parallel. The BioCAD software exported the images (.STL) to compare and verify deviations of the analogs on the X, Y, and Z axes. The nonparametric Kruskal-Wallis test and the two-way ANOVA on ranks with a post hoc Tukey test analyzed the data with 5% significance. Results: No statistical differences were observed in the accuracy between the extraoral scanners (p=0.0806). However, when analyzing only the components, component D was more accurate when scanned with Ceramill Map400+ compared with AutoScan DS200+ (p<0.001) and with E2 (p=0.002). Conclusions: All extraoral systems assessed showed digitalization accuracy but with more deviations in angled implants. The Ceramill Map400+ scanner showed the best results for the digital impression of a complete arch

OBJECTIVE@#To analyze the cement flow in the abutment margin-crown platform switching structure by using the three-dimensional finite element analysis, in order to prove that whether the abutment margin-crown platform switching structure can reduce the inflow depth of cement in the implantation adhesive retention.@*METHODS@#By using ANSYS 19.0 software, two models were created, including the one with regular margin and crown (Model one, the traditional group), and the other one with abutment margin-crown platform switching structure (Model two, the platform switching group). Both abutments of the two models were wrapped by gingiva, and the depth of the abutment margins was 1.5 mm submucosal. Two-way fluid structure coupling calculations were produced in two models by using ANSYS 19.0 software. In the two models, the same amount of cement were put between the inner side of the crowns and the abutments. The process of cementing the crown to the abutment was simulated when the crown was 0.6 mm above the abutment. The crown was falling at a constant speed in the whole process spending 0.1 s. Then we observed the cement flow outside the crowns at the time of 0.025 s, 0.05 s, 0.075 s, 0.1 s, and measured the depth of cement over the margins at the time of 0.1 s.@*RESULTS@#At the time of 0 s, 0.025 s, 0.05 s, the cements in the two models were all above the abutment margins. At the time of 0.075 s, in Model one, the gingiva was squeezed by the cement and became deformed, and then a gap was formed between the gingiva and the abutment into which the cement started to flow. In Model two, because of the narrow neck of the crown, the cement flowed out from the gingival as it was pressed by the upward counterforce from the gingival and the abutment margin. At the time of 0.1 s, in Model one, the cement continued to flow deep inside with the gravity force and pressure, and the depth of the cement over the margin was 1 mm. In Model two, the cement continued to flow out from the gingival at the time of 0.075 s, and the depth of the cement over the margin was 0 mm.@*CONCLUSION@#When the abutment was wrapped by the gingiva, the inflow depth of cement in the implantation adhesive retention can be reduced in the abutment margin-crown platform switching structure.

ABSTRACT The immediate rehabilitation of the posterior region of the mandible with dental implants, expands the set of possible actions for the dental surgeon, in the face of tooth loss. The purpose of this study is to describe two cases of single rehabilitation of mandibular molars with hybrid internal tapered implants indicated for immediate loading in post-extraction socket, associated with the maintenance of the prosthetic and peri-implant space. Implants with macrogeometry and surface treatment (Hydrophilic) were used to optimize primary stability, as well as the type of internal connection and prosthetic abutments that allows better accommodation and healing of adjacent tissues, and protection of the bone graft. Different methodologies of surgical guides did not influence the final result. The predictability of associating techniques and components can be observed in the 2-year follow-up. The initial planning combined with the new technologies enable to achieve stable and functional final restorations.


RESUMO A reabilitação imediata da região posterior da mandíbula com implantes dentários, amplia o conjunto de ações possíveis ao cirurgião dentista, frente as perdas dentárias. O intuito desse estudo é descrever dois casos de reabilitação unitária de molares inferiores com implantes cônicos internos híbridos indicados para carregamento imediato em alvéolos pós-extração, associado a manutenção do espaço protético e peri-implantar. Foram utilizados implantes com macrogeometria e tratamento de superfície que otimizassem a estabilidade primária (Hidrofílicos), bem como o tipo de conexão interna e abutments protéticos que permitisse melhor acomodação e cicatrização dos tecidos adjacentes, e proteção do enxerto ósseo. Diferentes metodologias de guias cirúrgicos não influenciaram o resultado final. A previsibilidade da associação de técnicas e componentes pode ser observada no follow-up de 2 anos. O planejamento inicial aliado às novas tecnologias permite restaurações finais estáveis e funcionais.

Aim: To evaluate the impact of xerostomia, edentulism, use of dental prosthesis, and presence of chronic diseases on quality of life in relation to oral health in institutionalized elderly individuals. Methods: This is a cross-sectional study. A questionnaire was administered containing the following instruments: Oral Health Impact Profile (OHIP-14), which measures the quality of life related to oral health; the Summated Xerostomia Inventory questionnaire (SXI-PL) for evaluation of xerostomia, sociodemographic data, clinical description, and patient-reported factors was assessed (edentulism, use of dental prostheses, and chronic diseases). Results: Most elderly individuals did not have any teeth in their mouths and used dental prosthesis. The impact on quality of life, considering the mean of the OHIP-14 scores, was positive in 58.3% of the elderly. Those who used a dental prosthesis were three times more likely to have their oral health negatively impacted (OR=3.09; 95%CI =1.17 8.11), compared to those who did not use, and individuals with xerostomia were more likely to have their oral health negatively impacted (OR=1.57; 95%CI=1.25-1.98) compared to those without xerostomia. There was no difference in the quality of life of individuals with and without chronic diseases. Conclusions: The feeling of dry mouth and use of dental prostheses negatively impacted the quality of life in relation to oral health of the elderly
